What are your office hours and appointment policies?

Our office hours are Monday through Friday from 8:30 a.m. to 5:00 p.m.  It is often helpful to our patients to make a list of questions for the doctor to answer during the examination.

If you have had x-rays or MRI's, the receptionist will ask you to bring them with you to your appointment.


If you are unable to keep your appointment or are going to be late, please call as soon as possible.  This courtesy allows us to be of service to other patients.  There will be a charge for broken appointments for which you do not appear and have not canceled.


What should I bring to my appointment?

Please be sure to bring your driver's license, insurance card, referral if required by your insurer, any previous x-rays or MRI films and reports, and payment.  Please make sure to always inform us of any changes in your contact or insurance information.

We ask new patients to arrive 20 minutes prior to their appointment to allow for parking and completion of paperwork o
r you can complete your paperwork online, print it, and bring it with you.  Paperwork

A parent or guardian must accompany minors under the age of 18 unless prior arrangements have been made.


If another physician has referred you here, please see that he or she sends your records.  The physician who referred you will receive a letter discussing your problem and our treatment plan.  They will be kept informed of you progress when indicated.




What are your financial policies?

Payment is expected at the time of service.  Payment may be made by cash, check, money orders, VISA, MasterCard, American Express, or Discover.

Our fees may vary with the time spent with you as well as the complexity of the evaluation or surgical procedure.  Please always feel free to discuss your fee before your visit or prior to any surgical procedure.


A fee will be charged for any forms we complete or reports we create for you; this charge will vary according to the amount of time involved in their preparation.


What are your billing policies?

We are pleased to file your insurance claim for you.  Please be prepared with your driver's license, insurance card, policy numbers, addresses and any other information we may need.

While we are happy to file your claim with your insurance, please understand that we are unable to wait indefinitely for payment by your carrier.  Payment for co-pays, deductibles, and co-insurance is due in full at the time of service.


How do I get a refill on my prescription?

Please contact your pharmacy for refills.  They will contact our office for approval.  Please note that we are unable to refill medications on the weekends.

What are your practices on medications and surgery?

If you have any questions about the use or side effects of your medication, please ask your physician or your pharmacist.  Always discontinue a medication that makes you ill and call the doctor.  Please give our office at least 24 hours notice in advance of needing a prescription refill.  No medications will be refilled over a weekend or holiday.

Anesthesia and hospital fees for surgery are not under the control of this office.  Please contact those health care providers directly with any questions.
